Our portal allows user to:

  8. Check appeal status
  9. Update employer’s information
Individual
  1. Copy of your NRIC for Malaysian / Copy of your passport if you are non-Malaysian
  2. Copy of calling visa/ visit pass of your worker
Company
  1. Copy of your company's registration documents (Form 9 or equivalent)
  2. Copy of the NRIC/Passport of contact person
  3. Copy of the letter authorizing the contact person to perform worker registrations
  4. Copy of calling visa for one worker
We only accept images with extension (jpg, pdf, png) for registration.
The maximum upload file size is 10MB.
At the moment, only Google Chrome is supported.
You may register each company as a separate employer. Each company must have a unique ROC company number. However, subsidiary company's first-time registration must be done at any FOMEMA branches.
It will take up to 2 working days for FOMEMA to process your employer registration request. Please note that working days may differ by state in Malaysia.
You may contact our Customer Service & Support Department during business hours to retrieve your login credentials.
It means that your record has already exist with us. You may contact our Customer Service & Support Department during business hours to retrieve your login credentials.
Please refer to the previous medical examination form and use the employer code to log in. Fill in your employer code > leave the password blank > click on login > update your details and email for first time login > the reset password link will be sent to your email
To reset your password, click on “Forgot Your Password” found under the Login button. This will allow you to request for a password reset.
One email address can only be used for one employer code uniquely. You may not share the same email address for another employer.
Please email to cs@fomema.com.my the following document to update your email address. Official letter to request to change email address (the letter should indicate the following details):
  • New Email address
  • Employer's Contact Number
  • Employer's Signature
Please attach together with:
  • Copy of employer's IC (Individual)
  • Copy of SSM form 9 (Company)
  • Worker's permit for one worker
  • Worker's passport for one worker
The process will take up to 3 working days. Please note that working days may differ by state in Malaysia.
Once it has been updated, you will receive an email to reset your password.
There are 4 main steps to purchase medical form.
Step 1: Register Worker
Step 2: Make Payment
Step 3: Select your panel clinic
Step 4: Print medical form
Company: Go to the “Register Worker” > find the worker you wish to register > tick on the worker check box > click “Add to List”> repeat until you have added all your worker in the list > click view list > click on “Buy FOMEMA”
Individual: Go to the “Register Worker” > find the worker you wish to register > tick on the worker check box > click on “Buy FOMEMA”
The csv template and batch upload specification can be obtained upon click on bulk upload new worker.
Go to “Register Worker” > find your worker > click on amendment > change the information > upload copy of passport and copy of permit > submit your amendment.
We only accept images with extension (jpg, pdf, png) for amendment request.
If the worker’s data has been verified with JIM (Jabatan Imigresen Malaysia), please proceed for amendment manually.

  1. Download & fill in Request to Amend Foreign Worker's Information form at https://www.fomema2u.com.my/forms/
  2. Copy of passport (old & new for updating passport number)
  3. Copy of work permit
The amendment process will take up to 3 working days. Please note that working days may differ by state in Malaysia.
Go to the “Register Worker”> find your worker > refer to the “Amendment Status” Column You will also receive an email once the amendment is approved/ rejected
You may click on the profile icon on top of your right to update your information such as tel. no, address, bank account etc. You are not allowed to amend critical information such as Employer Type, Employer Name, NRIC/ROC and e-mail address online.
You may click “Payment” > click “Show” to review the foreign worker name before the payment is done
Payment can be done later even if the worker has been selected, just go to “Payment” > click “Show” to confirm on the worker details > click “Edit payment” > click “Pay now”
Go to “Payment” > refer to the status of payment
You may go to “Payment” > click on “Invoice” to generate the invoice for the following transaction made via FOMEMA web portal.
FOMEMA Global is the authorized collection agent for FOMEMA Sdn Bhd.
Go to “Registration History” > tick the checkbox for the selected foreign worker > click “Select Clinic”
You may just fill in the field that you know. For the selection of doctor choose the state or town > click search and the list of clinics will be available for you to select the doctor and save accordingly.
You may click on the “Registration History” > refer to action column > click “Print”. The button will only appear after you have selected the clinic.
Go to “Registration History” > tick on the check box for the selected foreign worker > click refund > update employer profile (bank account number and bank name) The bank details should belong to the registered employer. Any representative account number is not allowed.
You may need to refund for the previous transaction that has been made via FOMEMA web portal and you have to perform a new medical registration with the full medical examination fee. For every refund request, RM27.00 per worker will be imposed as service fee and the balance will be credited to the respective employer's account.
Refund is not allowed after medical examination has been carried out or after the expiry date of the form.
Refund process will take place within 14 days from the date the request has been submitted.
Our portal allows user to purchase insurance only or purchase medical form and insurance together.
Three insurer options available at the moment.

Login> Go to medical result > select your search type (worker code or passport number) > click “Search”
You may need to refer to the medical examination form to obtain the employer code to login to the employer portal to check the medical result.
Fill in your employer code > leave the password blank > click on login > update your details for first time login > the reset password link will be sent to your email.
FOMEMA is the sole merchant for Boleh Wallet. Boleh Wallet offers payment method such as FPX, Credit/ Debit Card and e-wallet.
You need to download Boleh Wallet App through Apple App Store or Google Play Store.
Scan the QR code in FOMEMA online portal and select your payment method to pay.
All data shared with Boleh Wallet is secured and is end-to-end encrypted.
Boleh Wallet is a product of WorkerzDirect Sdn Bhd (Collection Agent) which is a financial payment solution for FOMEMA Sdn Bhd. Kiple Pay is our payment gateway provider.

The convenience fee is RM 3.00 per transaction

The QR code is only valid for 10 minutes. All payment has to be made within the validity of the QR code.
By going to “Register Worker” in FOMEMA online portal to make new payment.
Please avoid leaving the QR page once it has been generated. If you have accidentally closed the browser without copying down the QR payment URL, kindly wait for 10 minutes before reattempting to make a payment again in FOMEMA online portal.
Go to “Payment” and refer to payment status
Payment will be unsuccessful. The user will be redirected to FOMEMA online portal and go to “Register Worker” to make new payment.
You will be redirected to FOMEMA online portal to continue with clinic selection
Please contact FOMEMA customer service from Monday to Friday, 9.00am to 5.00pm to cancel your pending authorization
Wait for 10 minutes before reattempting to make a payment again in FOMEMA online portal
After 10 minutes, go to “Register Worker” in FOMEMA online portal to select correct worker and make new payment.
Go to “Payment” in FOMEMA online portal and click invoice.
